LASIK Candidacy in 2026: Who Is a Good Candidate and Who Is Not?

LASIK remains the most recognized name in laser eye surgery, and for good reason. Decades of refinement have made it one of the safest and most effective elective procedures available. But familiarity with the name doesn’t mean it suits every patient. 

Understanding what makes someone a strong candidate and what puts LASIK out of reach helps you approach your consultation with realistic expectations and the right questions.

How LASIK Works

LASIK corrects vision by reshaping the cornea, the clear front surface of the eye, using a laser. Your surgeon creates a thin flap in the outer corneal tissue, lifts it, applies the laser to reshape the underlying tissue, and then repositions the flap. The flap bonds without stitches and heals quickly. Most patients notice dramatically improved vision within 24 hours.

Modern LASIK uses wavefront-guided mapping and advanced excimer lasers that treat each eye based on its unique optical profile, not just a standard prescription. This precision has improved outcomes significantly compared to earlier generations of the procedure.

Who Makes a Strong LASIK Candidate

Several factors align to make someone well-suited for LASIK.

Adequate corneal thickness: LASIK removes corneal tissue to achieve the correction. You need enough tissue to complete the treatment safely and leave sufficient thickness remaining. Patients with naturally thicker corneas or lower prescriptions requiring less removal typically clear this threshold comfortably.

Prescription within treatable range: LASIK addresses nearsightedness up to roughly -10.00 or -12.00 diopters, farsightedness up to around +6.00, and astigmatism up to approximately 6.00 diopters, depending on corneal thickness. Prescriptions within these ranges, combined with sufficient tissue, generally qualify.

Good overall eye health: No active eye diseases, significant dry eye, keratoconus, or conditions affecting the cornea or retina. Healthy eyes without a history of certain conditions set the stage for predictable outcomes.

Age between roughly 18 and the mid-40s: Younger patients need a stable prescription. Patients in their late 40s and beyond face the added complication of presbyopia, at which point PRESBYOND or custom lens exchange often addresses the full picture more effectively than standard LASIK.

Who Is Not a Good Candidate

LASIK isn’t the right fit for every patient, and a thorough consultation will identify the reasons clearly.

Thin or irregular corneas represent the most common disqualifier. Patients with keratoconus, a condition where the cornea gradually thins and bulges, cannot safely undergo LASIK. 

Even patients without keratoconus but with borderline corneal thickness may be better served by SMILE, which causes less corneal disruption, or EVO ICL, which doesn’t touch the cornea at all.

Prescriptions outside the treatable range point toward EVO ICL instead. Patients with high myopia in particular often find that EVO ICL delivers better visual quality at their prescription level than laser surgery can achieve.

Significant dry eye creates complications both during healing and for long-term comfort. LASIK can worsen pre-existing dry eye, and patients with moderate to severe symptoms often get better outcomes with EVO ICL or SMILE.

Unstable prescription means waiting, not ruling out surgery altogether. If your refraction has shifted in the past year, your surgeon will recommend monitoring it for another year or two before proceeding.

Pregnancy and breastfeeding temporarily affect corneal shape and prescription stability. Surgery during this period produces less predictable results, so most surgeons advise waiting until hormonal changes have settled.

Certain systemic conditions, including autoimmune disorders and conditions affecting healing, can influence candidacy. This doesn’t automatically disqualify someone, but it requires careful evaluation.

What Happens If LASIK Isn’t Right for You
